Clinical study on the combined use of intra-abdominal pressure monitoring, Dachengqi decoction, and hot ironing therapy for gastrointestinal function recovery in elderly patients after hip fracture surgery
腹内压监测联合大承气汤与热熨治疗在老年患者髋部骨折术后胃肠功能恢复的临床研究

摘要

目的 观察腹内压监测配合大承气汤内服联合中药热熨，干预老年髋部骨折术后胃肠功能紊乱的实际效果。方法 选取2024年1月-2025年1月本院收治老年髋部骨折手术患者60例，随机分为对照组与观察组。均30例，对照组采用术后常规护理与胃肠基础调理，观察组在常规干预基础上开展腹内压动态监测，配合大承气汤口服、神阙穴中药热熨联合治疗，两组干预时长均为7天，对比两组胃肠功能恢复时间、术后便秘发生情况、安全性评估以及临床疗效。结果 观察组各项胃肠功能恢复时间均短于对照组；术后便秘发生率更低，不良反应发生率更低，整体疗效显著优于对照组；安全优势更为明显（P＜0.05）。结论 腹内压监测联合中药内服外治用于老年髋部骨折术后患者，可有效促进胃肠功能恢复，降低便秘发生概率，不良反应少，安全可靠，适合临床推广应用。

Abstract

Objective To observe the actual effect of intra-abdominal pressure monitoring combined with oral administration of Dachengqi decoction and traditional Chinese medicine hot ironing in intervening in gastrointestinal dysfunction after hip fracture surgery in elderly patients.
Methods Sixty elderly patients who underwent hip fracture surgery in our hospital from January 2024 to January 2025 were randomly divided into a control group and an observation group. There were 30 cases in each group. The control group received routine postoperative care and basic gastrointestinal regulation, while the observation group received dynamic monitoring of intra-abdominal pressure on the basis of routine intervention, combined with oral administration of Dachengqi Tang and traditional Chinese medicine hot ironing treatment at Shenque acupoint. The intervention duration for both groups was 7 days. The recovery time of gastrointestinal function, incidence of postoperative constipation, safety evaluation, and clinical efficacy were compared between the two groups.
Results The recovery time of various gastrointestinal functions in the observation group was shorter than that in the control group; The incidence of postoperative constipation is lower, the incidence of adverse reactions is lower, and the overall efficacy is significantly better than that of the control group; The safety advantage is more significant (P<0.05).
Conclusion   The combination of intra-abdominal pressure monitoring and traditional Chinese medicine oral and external treatment for elderly patients with hip fractures after surgery can effectively promote gastrointestinal function recovery, reduce the probability of constipation, have fewer adverse reactions, are safe and reliable, and are suitable for clinical promotion and application.
