Application effect of emergency nursing in patients with craniocerebral trauma based on improved early warning score of emergency criticality index
基于急诊危重度指数改良预警评分急诊护理在颅脑外伤患者中的应用效果

摘要

目的 探究基于急诊危重度指数构建的改良预警评分急诊护理模式应用于颅脑外伤患者中的临床实际效果。方法 选取2022年1月-2025年1月本院收治的56例颅脑外伤患者为研究对象，采用随机数字表法分为对照组与观察组，各28例。对照组实施急诊常规急救护理，观察组在常规护理基础上开展基于急诊危重度指数改良预警评分的规范化急诊护理。对比两组患者护理前后美国国立卫生研究院卒中量表（NIHSS）、格拉斯哥昏迷量表（GCS）、卡氏功能状态量表（KPS）评分变化，同时统计两组患者护理期间并发症发生情况。结果 护理前，两组各项量表评分对比无统计学差异（P＞0.05）；护理后，两组各项评分均较护理前改善，观察组NIHSS评分低于对照组，GCS评分、KPS评分均高于对照组，差异具有统计学意义（P＜0.05）；观察组并发症总发生率显著低于对照组（P＜0.05）。结论 将急诊危重度指数改良预警评分急诊护理用于颅脑外伤患者急诊救治中，可精准评估患者病情危重程度，规范急诊分级救治与转运流程，有效改善患者神经缺损症状、意识状态与躯体活动功能，减少临床并发症发生，提升急诊整体救治护理水平，适合临床广泛推广使用。

Abstract

Objective To explore the clinical effect of the improved early warning score emergency nursing model based on emergency severity index in patients with craniocerebral trauma.
Methods From January 2022 to January 2025, 56 patients with craniocerebral trauma in our hospital were selected as the research object, and randomly divided into control group and observation group, with 28 cases in each group. The control group carried out routine emergency care, while the observation group carried out standardized emergency care based on the improved early warning score of emergency criticality index on the basis of routine care. The score changes of National Institutes of Health Stroke Scale (NIHSS), Glasgow Coma Scale (GCS) and Karn of sky Functional Status Scale (KPS) were compared between the two groups before and after nursing, and the complications of the two groups during nursing were also counted.
Results Before nursing, there was no statistical difference between the two groups (P > 0.05). After nursing, all the scores of the two groups were improved. The NIHSS score of the observation group was lower than that of the control group, while the GCS score and KPS score were higher than that of the control group, with statistical significance (P < 0.05). The total incidence of complications in the observation group was significantly lower than that in the control group (P < 0.05).
Conclusion   Emergency nursing with improved early warning score of emergency severity index can accurately assess the severity of patients with craniocerebral trauma, standardize emergency graded treatment and transport process, effectively improve patients’ neurological deficit symptoms, conscious state and physical activity function, reduce the occurrence of clinical complications, and improve the overall level of emergency care, which is suitable for clinical application.
