The impact of the 5A nursing model guided by self-management on postoperative patients with malignant tumors in hepatobiliary and pancreatic surgery
自我管理导向下5A护理模式对肝胆胰外科恶性肿瘤术后患者的影响

摘要

目的 研究基于自我管理理念的5A护理模式对于肝胆胰外科恶性肿瘤病人术后的应用价值，分析该模式对于病人自我管理水平以及术后康复水平和生活质量的影响。方法 将2022年6月至2023年12月在我院肝胆胰外科接受手术治疗的恶性肿瘤病人随机分为观察组（51例）、对照组（51例），对照组给予常规护理，在此基础上观察组进行基于自我管理的5A护理模式（询问Ask、建议Advise、评估Assess、协助Assist、安排Arrange）干预。对比两组患者术后自我管理能力、术后并发症发生情况、护理满意度、生活质量评分。结果 观察组患者术后自我管理能力评分、生活质量各领域评分均明显高于对照组，术后并发症发生率低于对照组，护理满意度高于对照组，两组间差异均有统计学意义（P＜0.05）。结论 采取自我管理指导下的5A护理模式可以明显提高对肝胆胰外科恶性肿瘤术后患者的自我管理能力，促进术后康复，提高患者的生活质量，值得临床推广应用。

Abstract

Objective To study the application value of the 5A nursing model based on self-management for postoperative patients with malignant tumors in hepatobiliary and pancreatic surgery, and to analyze the impact of this model on the patients' self-management level, postoperative recovery level and quality of life.
Methods From June 2022 to December 2023, patients with malignant tumors who underwent surgery in our hospital were randomly divided into the observation group (51 cases) and the control group (51 cases). The control group received routine care, and the observation group was intervened with the 5A nursing model based on self-management (asking Ask, advising Advise, assessing Assess, assisting Assist, arranging Arrange). The postoperative self-management ability, postoperative complication occurrence, nursing satisfaction, and quality of life scores of the two groups were compared.
Results The postoperative self-management ability scores and scores in all domains of quality of life of the observation group were significantly higher than those of the control group, the postoperative complication rate of the observation group was lower than that of the control group, and the nursing satisfaction of the observation group was higher than that of the control group. The differences between the two groups were statistically significant (P < 0.05).
Conclusion   Adopting the 5A nursing model guided by self-management can significantly improve the self-management ability of postoperative patients with malignant tumors in hepatobiliary and pancreatic surgery, promote postoperative recovery, and improve the quality of life of patients. It is worthy of clinical promotion and application.
