基于喂养-睡眠时机的体位干预对早产儿呼吸节律及氧合状态的影响研究

摘要

目的 探讨基于喂养-睡眠时机的体位干预对早产儿呼吸节律及氧合状态的影响。方法 选取我院新生儿重症监护室(NICU)2025年3月至2026年1月收治的早产儿100例,按随机数字表法分为对照组(n=50)与观察组(n=50)。对照组实施常规体位护理(仰卧、侧卧位定时交替),观察组在喂养前、喂养时及喂养后实施基于喂养-睡眠时机的体位干预方案。比较两组早产儿的呼吸暂停发生率、呼吸频率、动脉血氧分压(PaO₂)、动脉血二氧化碳分压(PaCO₂)及经皮血氧饱和度(SpO₂)。结果 干预后,观察组呼吸暂停发生率显著低于对照组(P<0.05);观察组呼吸频率低于对照组(P<0.05)。观察组PaO₂高于对照组,PaCO₂低于对照组(P<0.05)。结论 基于喂养周期与睡眠生理的时机化体位干预,能有效改善早产儿呼吸节律、降低呼吸暂停发生率、提高氧合水平。

Abstract

Objective To explore the effects of position intervention based on feeding-sleep timing on respiratory rhythm and oxygenation status in preterm infants.
Methods A total of 100 preterm infants admitted to the Neonatal Intensive Care Unit (NICU) of our hospital from March 2025 to January 2026 were selected and randomly divided into a control group (n=50) and an observation group (n=50) using a random number table. The control group received routine position care (supine and lateral positions alternated at regular intervals), while the observation group implemented a position intervention program based on feeding-sleep timing before, during, and after feeding. The incidence of apnea, respiratory rate, arterial oxygen pressure (PaO₂), arterial carbon dioxide pressure (PaCO₂), and peripheral oxygen saturation (SpO₂) were compared between the two groups.
Results After the intervention, the incidence of apnea in the observation group was significantly lower than that in the control group (P<0.05); the respiratory rate in the observation group was lower than that in the control group (P<0.05). PaO₂ in the observation group was higher than in the control group, and PaCO₂ was lower than in the control group (P<0.05).
Conclusion   Timing-based position intervention according to feeding cycles and sleep physiology can effectively improve respiratory rhythm, reduce the incidence of apnea, and enhance oxygenation in preterm infants.
