腕踝针联合阿片类药物治疗乳腺癌癌痛的研究进展

摘要

腕踝针疗法是在传统经络理论基础上发展而来、融合现代针刺技术的一种特殊疗法，目前在癌性疼痛的临床治疗中已有较为广泛的实际应用。本文将从以下几个层面展开系统论述：首先阐释该疗法的理论渊源与核心机制，其次介绍其标准化的操作流程与规范，最后重点分析其在癌性疼痛管理中的临床应用现状——尤其聚焦于与阿片类镇痛药物联合应用于乳腺癌患者时所能带来的协同增效作用与临床获益。现有临床数据表明，腕踝针单独应用时，可有效降低患者的疼痛评分，并减少爆发性疼痛的发生频率，但其镇痛强度及作用维持时间仍存在一定局限性，尚不能满足重度癌痛的长期控制需求。而将其与阿片类药物联合使用后，整体镇痛效果呈现显著提升，疼痛缓解持续时间明显延长，同时阿片类药物相关不良反应（如恶心、便秘、嗜睡等）的发生率与严重程度均有所降低。患者不仅在生理层面获得更优的疼痛控制，其生活质量和心理状态亦同步改善。此外，该联合方案在缓解焦虑、抑郁等负性情绪以及改善睡眠质量方面，也显示出一定的辅助疗效。目前尚缺乏针对不同癌种之间腕踝针疗效的头对头比较研究，进一步的临床研究仍有待完善。然而，在乳腺癌患者群体中，现有证据已表明腕踝针联合阿片类药物具有良好的安全性与有效性。未来研究应着力于开展多中心、大样本的随机对照试验，系统评估腕踝针与不同类型镇痛药物组合的个体化治疗策略，同时深入探讨其潜在的神经生理与分子镇痛机制，以期为癌痛的综合管理提供更具循证依据的干预方案。

Abstract

Wrist-ankle acupuncture is a specialized therapeutic modality grounded in classical meridian theory and integrated with contemporary acupuncture principles. It has been widely adopted in clinical practice for the management of cancer-related pain. This article offers a systematic analysis from three key perspectives: first, it delineates the theoretical foundations and core mechanisms underlying this approach; second, it details standardized operational protocols and clinical guidelines; and third, it critically examines its current applications in cancer pain management—particularly highlighting the synergistic analgesic effects and clinical advantages observed when combined with opioid analgesics in patients with breast cancer. Available clinical evidence indicates that wrist-ankle acupuncture administered as monotherapy can effectively reduce numerical rating scale (NRS) pain scores and decrease the frequency of breakthrough pain episodes. However, its analgesic intensity and duration of effect are inherently limited, rendering it insufficient for sustained control of moderate-to-severe cancer pain. In contrast, when integrated with opioid therapy, overall analgesic efficacy is significantly enhanced, with demonstrable prolongation of pain relief. Importantly, the incidence and severity of opioid-associated adverse effects—including nausea, constipation, and sedation—are meaningfully attenuated. Consequently, patients not only achieve superior physiological pain control but also report measurable improvements in health-related quality of life and psychological well-being. Furthermore, this integrative strategy confers adjunctive benefits in mitigating anxiety and depression, as well as in enhancing sleep quality. To date, head-to-head comparative studies evaluating the efficacy of wrist-ankle acupuncture across diverse cancer types remain scarce, underscoring the need for further rigorous clinical investigation. Nevertheless, existing evidence robustly supports the safety and efficacy of combining wrist-ankle acupuncture with opioid analgesics specifically in breast cancer populations.
