摘要
目的 探讨信息化平台延续护理在糖尿病患者血糖管控中的应用效果。方法 选取2025年1月-2025年12月期间本院收治的80例糖尿病患者，采用随机数字表法分组，各40例。对照组给予常规出院指导和电话随访，观察组采用信息化平台延续护理。比较两组患者的血糖控制情况、自我管理行为评分。结果 观察组在血糖控制方面优于对照组（P＜0.05），自我管理行为评分高于对照组（P＜0.05）。结论 信息化平台延续护理能显著改善糖尿病患者的血糖水平，提升自我管理能力，值得推广。

Abstract
Objective To explore the application effect of information platform continuous nursing in blood glucose control of diabetes patients. Methods 80 patients with diabetes admitted to our hospital from January 2025 to December 2025 were selected and divided into 40 cases each by random number table. The control group received routine discharge guidance and telephone follow-up, while the observation group continued nursing care through an information platform. Compare the blood glucose control and self-management behavior scores of two groups of patients. Results The observation group showed better blood glucose control than the control group (P<0.05), and the self-management behavior score was higher than that of the control group (P<0.05). Conclusion Continuous nursing on information platform can significantly improve blood glucose level of patients with diabetes, improving self-management ability, is worth promoting.
