The application effect of community nursing services in the management of chronic diseases among the elderly in the community
社区护理服务在社区老年慢性病管理中的应用效果

摘要

目的 将社区护理服务应用在社区老年慢性病患者管理中，探究其护理价值。方法 2023年1月-2024年1月期间在本社区中选择65岁及以上的老年慢性病患者80例为范例。以电脑随机数字表法分组。观察组（n=40）实施社区护理服务干预，对照组（n=40）实施基础社区健康管理干预。比较两组的护理成效。结果 干预前两组患者的自我管理评分比较无统计学意义（P＞0.05）。观察组患者干预6个月后，自我管理评分各维度均高于对照组，组间比较差异有统计学意义（P＜0.05）。观察组并发症占比低于对照组，有统计学意义（P＜0.05）。结论 社区护理服务方法的应用，能够有效提升患者自我管理能力，降低并发症的发生风险。

Abstract

Objective To apply community nursing services in the management of elderly patients with chronic diseases in the community and explore its nursing value.
Methods From January 2023 to January 2024, 80 elderly patients aged 65 and above with chronic diseases in this community were selected as the sample. They were divided into groups using the computer random number table method. The observation group (n=40) received community nursing service intervention, while the control group (n=40) received basic community health management intervention. The nursing effects of the two groups were compared.
Results Before the intervention, there was no statistically significant difference in the self-management scores of the two groups (P＞0.05). After 6 months of intervention, the self-management scores of each dimension of the observation group were higher than those of the control group, and the difference between the groups was statistically significant (P＜0.05). The proportion of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P＜0.05).
Conclusion   The application of community nursing service methods can effectively improve patients' self-management ability and reduce the risk of complications.
