The clinical efficacy of open-chain and closed-chain combined training in the treatment of shoulder subluxation after stroke
开链与闭链联合训练治疗脑卒中肩关节半脱位的临床疗效

摘要

目的 研究开链和闭链训练用于脑卒中肩关节半脱位治疗的可行性。方法 遴选60例2025年1月20日-2025年4月20日在该院收治的脑卒中肩关节半脱位患者，划分患者为联合组与对照组，为对照组患者提供开链康复训练，在开链康复训练基础上，为联合组增加闭链训练。结果 训练4周、8周时，所有患者上肢运动功能、肩部疼痛均得到改善，尤其是联合组FMA评分提升幅度更大、VAS评分下降幅度更大，P<0.05，差异存在统计价值；联合组治疗有效占比96.67%，对照组治疗有效占比80.00%，对比可知，联合组疗效更佳，数据差异存在统计价值（P<0.05）。结论 开链闭链训练用于脑卒中后肩关节半脱位治疗的效果显著，可有效改善患者上肢运动功能与肩部疼痛症状，可别广泛用于肩关节半脱位治疗。

Abstract

Objective To study the feasibility of open-chain and closed-chain training in the treatment of shoulder subluxation after stroke.
Methods Sixty patients with shoulder sudislocation due to stroke who were admitted to the hospital from January 20, 2025 to April 20, 2025 were selected and divided into the combined group and the control group. Open-chain rehabilitation training was provided for the patients in the control group, and closed-chain training was added to the combined group on the basis of the open-chain rehabilitation training.
Results At 4 and 8 weeks of training, the upper limb motor function and shoulder pain of all patients were improved. Especially in the combined group, the increase in FMA score was greater and the decrease in VAS score was greater, P<0.05, and the difference was of statistical value. The effective treatment rate of the combined group was 96.67%, while that of the control group was 80.00%. By comparison, it can be seen that the combined group had a better therapeutic effect, and the data difference had statistical value (P<0.05).
Conclusion   Open-chain and closed-chain training has a significant effect on the treatment of shoulder subluxation after stroke. It can effectively improve the upper limb motor function and shoulder pain symptoms of patients and can be widely used in the treatment of shoulder subluxation.
