Observation of health education and chronic disease management nursing in elderly people with hypertension in the community
健康教育与慢性病管理护理在社区老年高血压中的观察

摘要

目的 以社区老年高血压患者为对象，探究健康教育联合慢性病管理护理的应用效果。方法 本研究在2024年1月至12月期间从社区医院筛选出84例社区老年高血压患者作为研究样本。遵循随机化分组原则，将研究对象划分为两个组别：对照组（42例，接受常规护理）；研究组（42例，接受健康教育联合慢性病管理护理）。对两组患者的护理效果展开对比分析。结果 与对照组相比，研究组患者的收缩压、舒张压数值显著降低，且研究组患者的高血压疾病认知度评分也显著提升，差异有统计学意义（P＜0.05）。结论 针对社区老年高血压患者，实施健康教育协同慢性病管理护理的综合干预能够大幅提升患者对自身所患疾病的认知程度，并达成对血压指标的有效控制。

Abstract

Objective In this study, the application effect of health education combined with chronic disease management and nursing was explored in the elderly patients with hypertension in the community.
Methods This study screened 84 community-aged hypertensive patients from community hospitals from January to December 2024 as a study sample. Following the principle of randomization, the study subjects were divided into two groups: control group (42 cases, receiving usual care); Study group (42 cases, who received health education combined with chronic disease management care). The nursing effects of the two groups were comparatively analyzed.
Results Compared with the control group, the systolic blood pressure and diastolic blood pressure of the patients in the study group were significantly reduced, and the cognition score of hypertension in the study group was also significantly increased, and the difference was statistically significant (P<0.05).
Conclusion   For elderly hypertensive patients in the community, the implementation of comprehensive intervention of health education and chronic disease management and nursing can greatly improve patients' awareness of their own diseases and achieve effective control of blood pressure indicators.
