The impact of continuity of care intervention based on WeChat platform on maintenance hemodialysis patients
基于微信平台的延续性护理干预对维持性血液透析患者的影响

摘要

目的 研究微信平台在维持性血液透析患者中实施延续性护理干预的效果与影响。方法 选择我院于2023年1月-2024年12月收治的维持性血液透析患者100例，将其按随机分组方法分为对照组（50例，采取常规护理方法）和观察组（50例，采取基于微信平台的延续性护理干预），对两组的护理质量评分、患者自我管理能力评分、生活质量评分以及护理满意度进行对比和评估。结果 两组在护理后均取得一定效果，但观察组患者的自我管理能力评分、生活质量评分以及护理满意度均高于对照组（P＜0.05）。观察组护理质量评分高于对照组（P＜0.05）。结论 采用微信平台对维持性血液透析患者进行延续性护理干预，能有效提升患者的自我管理能力及生活质量，成效显著。提升其护理满意度。有较高应用价值。

Abstract

Objective Study the effect and impact of implementing continuity of care interventions on maintenance hemodialysis patients using WeChat platform.
Methods Select 100 maintenance hemodialysis patients admitted to our hospital from January 2023 to December 2024, and randomly divide them into a control group (50 cases, receiving routine nursing methods) and an observation group (50 cases, receiving continuous nursing intervention based on WeChat platform). Compare and evaluate the nursing quality score, patient self-management ability score, quality of life score, and nursing satisfaction of the two groups.
Results Both groups achieved certain results after nursing, but the self-management ability score, quality of life score, and nursing satisfaction of the observation group were higher than those of the control group (P<0.05). The nursing quality score of the observation group was higher than that of the control group (P<0.05).
Conclusion   The use of WeChat platform for continuous nursing intervention in maintenance hemodialysis patients can effectively improve their self-management ability and quality of life, with significant results. Improve their nursing satisfaction. Has high application value.
