Study on health education and chronic disease management and nursing in elderly hypertension in community
健康教育与慢性病管理护理在社区老年高血压中的研究

摘要

目的 分析健康教育与慢性病管理护理在社区老年高血压中的研究。方法 选择我院于2023年1月-2024年1月内收治的35例老年高血压患者为研究对象，将其按随机分组方法分为对照组（17例，采取常规护理管理）和观察组（18例，采取健康教育与慢性病管理护理），对两组患者的焦虑程度评分、抑郁程度评分、舒张压、收缩压、疾病认知水平评分及护理满意度进行对比与评估。结果 观察组患者的舒张压、收缩压、焦虑程度评分、抑郁程度评分均低于对照组（P＜0.05）。观察组的血压正常范围、相关并发症、高危因素、健康生活方式等疾病认知水平评分均高于对照组（P＜0.05）。观察组患者的护理满意度高于对照组（P＜0.05）。结论 在对社区老年高血压患者实施健康教育与慢性病管理护理可取得明显的治疗效果，显著改善患者的负面情绪，降低血压，提高护理满意度，值得推广与应用。

Abstract

Objective To analyze the study of health education and chronic disease management and nursing in elderly people with hypertension in community.
Methods 35 elderly patients with hypertension admitted to our hospital from January 2023 to January 2024 were selected as the study objects, and they were randomly divided into control group (17 cases, receiving routine nursing management) and observation group (18 cases, receiving health education and chronic disease management and nursing). The scores of anxiety degree, depression degree, diastolic blood pressure, systolic blood pressure, disease cognition level and nursing satisfaction of the two groups were compared and evaluated.
Results The diastolic blood pressure, systolic blood pressure, anxiety degree score and depression degree score of observation group were lower than those of control group (P < 0.05). Blood pressure normal range, related complications, risk factors, healthy lifestyle and other disease cognition scores in the observation group were higher than those in the control group (P < 0.05). The nursing satisfaction of observation group was higher than that of control group (P < 0.05).
Conclusion   The implementation of health education and chronic disease management and nursing for elderly hypertension patients in communities can achieve obvious therapeutic effects, significantly improve patients' negative emotions, reduce blood pressure, and improve nursing satisfaction, which is worthy of promotion and application.
