健康管理模式在社区老年慢性病患者中的应用

摘要

目的 探究社区老年慢性病患者管理中采取健康管理模式的效果。方法 此次研究所选取的研究对象为本院在2020年2月-2021年2月期间收治的88例社区老年慢性病患者。通过随机数字表法分为参考组与实验组。每组各44例。其中参考组给予常规管理,实验组采取健康管理模式。以临床指标达标率、遵医行为及满意度评价2组管理效果。结果 实验组临床指标达标率高于参考组(P<0.05),实验组遵医行为高于参考组(P<0.05)。实验组满意度高于参考组(P<0.05)。结论 社区老年慢性病患者管理中采取健康管理模式可有助于改善患者的各项指标,促进其遵医行为,有助于患者的病情康复,从而患者较为满意。此种管理方法可积极推广。

Abstract

Objective To explore the effect of health management mode in the management of elderly patients with chronic diseases in the community.
Methods The research object selected in this study was 88 elderly patients with chronic diseases in the community who were admitted to our hospital from February 2020 to February 2021. They were divided into reference group and experimental group by random number table. There were 44 cases in each group. The reference group was given routine management, and the experimental group was given health management mode. The management effect of the two groups was evaluated by clinical indicators, compliance behavior and satisfaction.
Results The indexes of the experimental group were better than those of the reference group (P. The satisfaction of the experimental group was higher than that of the reference group (P<0.05).
Conclusion   The adoption of health management model in the management of elderly patients with chronic diseases in the community can help improve the indicators of patients, promote their compliance behavior, and help patients recover from illness, so that patients are more satisfied. This management method can be actively promoted.
